Cardiovascular eVects of mechanical ventilation

چکیده

The heart and lungs work closely to meet the tissues’ oxygen demands. If the balance between oxygen demand and supply becomes disturbed in critical illness, tissue hypoxia and cell death can rapidly result. An essential part of critical care is to maintain cardiopulmonary function with the help of pharmacotherapy, fluid management, and respiratory support. Paradoxically, interventions aimed at improving the function of one system can sometimes have undesirable eVects on the other and, although the pulmonary consequences of cardiac disease are well recognised, the influences of changes in pulmonary physiology on cardiac function are less well appreciated. Cardiopulmonary interactions (the eVects of spontaneous and mechanical ventilation on the circulation) were first documented in 1733, when Stephen Hales showed that the blood pressure of healthy people fell during spontaneous inspiration. Over a century later Kussmaul described pulsus paradoxus (the inspiratory absence of the radial pulse) in patients with tuberculous pericarditis. Cardiopulmonary interactions are present in health, and can be exaggerated or abnormal in the presence of disease. This article will provide an overview of this broad topic. By emphasising the underlying physiological principles and the influence of disease states upon these, we hope that respiratory support will then be tailored to the individual patient.

Comparison of the Effects of Open and Closed Endotracheal Suction on Cardiovascular and Ventilation Parameters for Patients Undergoing Mechanical Ventilation

Background and Objectives: Suction is a necessary and common method which aids in the release of secretion from the respiratory system for the patients who have undergone mechanical ventilation. One of the most common complications of suction is hypoxemia which causes dysrhythmia and death. The purpose of this study was to determine the effects of open and closed-endotracheal suction on cardiov...

The effect of protocol of weaning from mechanical ventilation on gastrointestinal function in people undergoing coronary artery bypass graft surgery (CABG)

Abstract Aim. The aim of this study was to examine the effect of protocol of weaning from mechanical ventilation on the gastrointestinal function in people undergoing CABG. Background. The most common problems after cardiac surgery in the intensive care unit are change in vital signs, respiratory problems, unsuccessful separation from mechanical ventilation, and gastrointestinal problems. Due t...

Long term eVects of non-invasive mechanical ventilation on pulmonary haemodynamics in patients with chronic respiratory failure

Background—It is not known whether long term nocturnal mechanical ventilation (NMV) reduces pulmonary hypertension in patients with chronic respiratory failure (CRF). Methods—Pulmonary haemodynamics, spirometric values, and gas exchange were studied in 33 patients requiring NMV due to CRF (20 with thoracic restriction, 13 with chronic obstructive pulmonary disease (COPD)) at baseline and after ...

Effects of the use of mechanical ventilation weaning protocol in the Coronary Care Unit: randomized study.

OBJECTIVE To compare mechanical ventilation weaning based on a protocol using the spontaneous breathing trial against mechanical ventilation weaning without a standardized protocol in heart patients. METHODS Prospective, open, randomized study. In 2006, 36 patients undergoing mechanical ventilation for over 24 hours were randomized into two groups: control group - eighteen patients whose mech...
